Series recording misses episodes

Tell the PVR to record all episodes of a series and most, but not all, are recorded!

We have noticed this problem with two series in particular: Heartbest (on Knowledge Saturday nights) and Midsomer Murders (on PBS Thursday nights.)

We have set the PVR to record all episodes of these series. Heartbeat occurs twice (Saturday night and, a few weeks later,  early Sunday morning) and only the time we want is recorded. Similarly for Midsomer Murders (Thursday evening and next week on Thursday afternoon). In the past both repeats would have been scheduled, so kudos to Shaw for fixing that problem.

Most episodes of the series are recorded, but not all. We have to review the upcoming recordings to make sure nothing has been missed. When we find an episode that is not scheduled to be recorded, we use the Guide to set it to be recorded. The Guide menu shows Change Series Options (so the episode is recognized as part of a series)  but the episode is not treated as part of the series for recording.

We thought this was a problem with too many concurrent recordings, but that is not the problem.

What is happening? More importantly, how can we resolve it?
